Good Faith Estimate Notice

Under the No Surprises Act, if you are uninsured or are not using insurance (self-pay) for an item or service, you have the right to receive a Good Faith Estimate of the expected charges.

Your right to a Good Faith Estimate

  • You can ask for a Good Faith Estimate before you schedule care.
  • If you schedule an item or service at least 3 business days in advance, you should receive a Good Faith Estimate.
  • A Good Faith Estimate is not a bill and is not a binding contract; actual charges may differ based on services provided.

How to request an estimate

Call (909) 591-8200, ask at any TotalCare location when scheduling or asking about costs, or contact us through our website scheduling options. Tell us you are uninsured or choosing not to use insurance and which services you want estimated.

Questions and disputes

If you receive a bill that is substantially higher than your Good Faith Estimate, federal rules may allow you to dispute the bill through the Patient-Provider Dispute Resolution process. Ask our billing team for help, and see federal consumer resources at https://www.cms.gov/nosurprises.
